Homemaker/Personal Care Homemaker/Personal care, known as HPC, supports a person to be more independent while meeting their daily living needs. Direct service providers help people with household chores and personal care, including, but not limited to: getting dressed and keeping clean, making food, managing money to pay household bills, cleaning and doing laundry, getting out and being part of the community, and helping people to get to medical and dental appointments or other health care services. HPC also includes support that improves a person's ability to express their opinions and choices. When to Apply this Service? The individual's service and support administrator, or SSA, will assess the individual for a need for Homemaker/Personal Care. If there is a need, the SSA will include it in the person's individual service plan. To find out if your loved one qualifies contact the Erie County Board of Developmental Disabilities at 419-626-0316 and ask to speak with the person in charge of intakes.
HPC Transportation ERLI offers a variety of transportation services. HPC transportation usually goes hand in hand with HPC services. HPC transportation is a service when an individual needs transportation to medical appointments, stores and other places in the community. This service is not to be confused with non-medical transportation.
Non-Medical Transportation Non-Medical transportation is offered here at Erie Residential Living. We have a 15+ passenger, wheelchair accessible bus that transports individuals to the following locations: Adult Day Support, Career Planning, Group Employment Support, Individual Employment Support, Vocational Habilitation, or to their place of employment. Erie Residential Living has openings for NMT transportation and are open to accepting new individuals.
